¿Qué es Saw Palmetto?
Saw palmetto is a low, spreading fan palm of the south-eastern United States, named for the saw-toothed stems of its leaves. Its dark, oily berries were eaten by Native Americans such as the Seminole, and in the 19th century American doctors prescribed them for urinary and reproductive complaints. Today the berry extract, rich in fatty acids and plant sterols (such as beta-sitosterol), is one of the most popular men's health supplements in Europe and North America.
It is mainly taken for the urinary symptoms of an enlarged prostate (benign prostatic hyperplasia, BPH). Early, smaller trials were encouraging, but larger and better-designed trials — and systematic reviews pooling them — found saw palmetto did not improve urinary symptoms more than placebo, even at high doses. It is very safe, so some men still try it, but expectations should be modest.

Urinary symptoms of an enlarged prostate
The main use. Older, smaller studies reported fewer night-time bathroom trips and better flow, but large placebo-controlled trials and Cochrane reviews found no meaningful benefit over placebo. Some European doctors still use specific standardised extracts, and research continues.
Anti-androgen activity in the lab
In test tubes, saw palmetto extracts mildly block 5-alpha-reductase, the enzyme that converts testosterone into the more potent DHT. The effect in people appears much weaker than that of prescription drugs such as finasteride.
Hair thinning (weak evidence)
Because of its effect on DHT, it is marketed for male- and female-pattern hair loss. Only a few small studies exist, with mixed quality; treat it as experimental.
Anti-inflammatory in prostate tissue
Lab and some small human studies suggest it may reduce inflammation in prostate tissue. The clinical significance is unclear.

Standardised extract
The form studied: 320 mg daily of a lipidosterolic extract (standardised to about 85–95% fatty acids and sterols), as one dose or 160 mg twice a day, with food. Give it at least 3 months before deciding whether it helps.

Urinary symptoms such as weak flow, urgency or frequent night-time urination can have several causes, including prostate cancer. Get a proper check before self-treating, and keep your doctor informed.
Whole berries
Dried berries can be made into a tea (1 tsp crushed berries simmered 10 minutes), but the active compounds are fat-soluble and barely dissolve in water. The taste is also famously unpleasant — soapy and rancid.
Combination products
Often sold with nettle root or pumpkin seed. Some combinations have been tested in small trials; evidence is no stronger than for saw palmetto alone.
⚠️ Precauciones
- Avoid in pregnancy and breastfeeding: its hormone-modulating effects could affect a developing baby, particularly male development. Not for use by children.
- Because it may act on sex hormones, it could in theory interfere with the contraceptive pill, hormone replacement therapy and hormone-based cancer treatments — check with your doctor.
- May slightly increase bleeding risk; tell your doctor if you take warfarin, aspirin, clopidogrel or other blood thinners, and stop it two weeks before surgery.
- It does not appear to change PSA test results much, but always tell your doctor you are taking it so any prostate testing is interpreted correctly.
- Generally well tolerated; mild stomach upset, headache and dizziness are the most common side effects. Rare reports of liver and pancreas problems exist, though a clear link has not been proven. People allergic to palm-family plants should avoid it.
