¿Qué es Milk Thistle?

Milk thistle is a tall, spiny Mediterranean thistle with purple flower heads and glossy leaves marbled with white veins — legend says from a drop of the Virgin Mary's milk, hence marianum. It was known to classical Greek and Roman writers, and by the 16th and 17th centuries European herbalists were recommending the seeds for liver and spleen complaints. The active complex, silymarin, is a group of flavonolignans (silybin being the main one) concentrated in the seed coat.

Silymarin is one of the most researched herbal extracts. In the lab and in animals it protects liver cells from toxins, reduces oxidative stress and dampens inflammation. Human trials in hepatitis, alcohol-related and fatty liver disease are numerous but mixed: some show improvements in liver enzymes, few show clear effects on hard outcomes such as survival. It is best seen as a very safe, possibly helpful supporting herb — not a liver repair kit, and never a licence to keep drinking.

Liver-protective in the lab

Silymarin stabilises liver cell membranes, boosts the antioxidant glutathione and slows scarring processes in laboratory and animal studies. How much of this carries over to people is still debated.

May improve liver enzymes in fatty liver

Several small trials in non-alcoholic fatty liver disease reported modest drops in liver enzymes and, in some, less fat on imaging. Weight loss and exercise remain far more powerful; milk thistle is at most an add-on.

Mushroom poisoning (hospital use)

An intravenous form of silibinin is used in some European hospitals for death cap (Amanita phalloides) poisoning, where it appears to block toxin uptake by the liver. This is emergency medical treatment — capsules at home are no substitute.

May help blood sugar

A handful of trials in people with type 2 diabetes found small improvements in fasting glucose and HbA1c with silymarin. Promising but preliminary, and relevant mainly as an interaction to watch if you take diabetes medication.

Antioxidant support

Silymarin is a potent antioxidant in test-tube studies. Whether that translates into general health benefits in healthy people is unproven.

Standardised capsules

The form used in trials: extract standardised to about 70–80% silymarin, typically 140 mg of silymarin two or three times daily with meals. Give it 4–8 weeks before judging any change in blood tests.

Ground seeds

Grind 1 tsp of whole seeds just before use and sprinkle on porridge, yoghurt or salads. Tasty and fibre-rich, but delivers far less silymarin than an extract.

Tea

Crush 1 tsp of seeds and simmer in a cup of water for 10–15 minutes. Silymarin dissolves poorly in water, so tea is gentle and traditional rather than a strong dose.

Tincture

Usually 1–2 ml up to three times daily in a little water. Alcohol-based tinctures are an odd choice for anyone taking it for their liver — look for glycerin-based versions if that matters to you.

⚠️ Precauciones

  • Pregnancy and breastfeeding: it has been used traditionally to encourage milk flow, but safety data for concentrated extracts is limited — ask your midwife or doctor first.
  • May lower blood sugar; if you take insulin or other diabetes medication, monitor your levels and tell your doctor.
  • Silymarin may affect how the liver processes some drugs (certain cancer drugs, statins, warfarin and others); the effect seems mild in practice, but check with a pharmacist if you take regular medication.
  • Avoid if you are allergic to the daisy family (ragweed, chrysanthemums, marigolds, daisies) — cross-reactions are possible.
  • Generally very well tolerated; the most common side effects are loose stools, bloating or mild stomach upset. Overdose toxicity is not known, but more is not better.
  • Plant extracts may have weak oestrogen-like activity; people with hormone-sensitive conditions (such as some breast cancers or endometriosis) should check with their doctor.

Will milk thistle "detox" my liver?

Your liver detoxifies itself; there is no good evidence that milk thistle speeds this up in healthy people. What it may do is offer some protection to liver cells under stress — which is a different and more modest claim.

Can it undo the effects of heavy drinking?

No. Trials in alcohol-related liver disease are mixed at best, and no herb protects you from continued heavy drinking. Cutting back is the treatment that works.

Is it safe to take long-term?

Trials lasting several years have reported a good safety record. If you have diagnosed liver disease, take it with your doctor's knowledge rather than instead of their care.
