Black pepper is the dried unripe fruit of a climbing vine from the Malabar Coast of south-west India. It drove centuries of the spice trade, was used as currency and tribute, and remains the world's most traded spice. Black, white and green peppercorns all come from the same plant, processed differently. Its bite comes from piperine; its aroma from volatile oils such as limonene and caryophyllene.
Pepper's most interesting property is that piperine slows the breakdown and removal of certain substances by the gut and liver, so more of them reach the bloodstream. That is why it is added to turmeric supplements to boost curcumin absorption — and why concentrated piperine can raise blood levels of some medicines. In a small, widely cited human study, taking piperine with curcumin (from turmeric) raised curcumin blood levels dramatically. This is the basis for pairing turmeric with black pepper, though the study used a concentrated piperine dose, so how much a normal grind of pepper helps is less certain.
Stimulates digestion
Pepper's pungency triggers saliva and digestive secretions, and it has been used in Ayurveda (as part of the "three pungents", trikatu) to kindle a sluggish appetite and digestion. Mostly traditional and animal evidence.
May help swallowing reflexes (early evidence)
Small Japanese studies found that sniffing black pepper oil improved the swallowing reflex in older people with swallowing difficulties. Interesting, but specialised and preliminary.

Piperine shows antioxidant and anti-inflammatory activity in lab and animal studies. Whether culinary pepper has meaningful effects of this kind in people is unknown.

- Cooking amounts are safe for almost everyone, including in pregnancy and breastfeeding. Avoid high-dose piperine supplements in pregnancy — they have not been tested for safety.
- Drug interactions: concentrated piperine (for example in turmeric capsules labelled with black pepper extract) can raise blood levels of some medicines, including phenytoin, propranolol, theophylline and drugs broken down by the liver enzyme CYP3A4. If you take regular medication, ask a pharmacist before using piperine supplements.
- Pepper can aggravate heartburn, reflux, gastritis and haemorrhoids in sensitive people; reduce it if it makes your symptoms worse.
- Inhaling large amounts of ground pepper can be dangerous; there are reports of serious and even fatal breathing emergencies in small children. Keep pepper shakers and bulk pepper away from toddlers.
- Allergy to black pepper is rare but possible. Pink peppercorns are an unrelated plant in the cashew family and may affect people with cashew or mango allergies.
